How to use this page

  1. Pick the email below that matches your situation.

You do not have to use all of this. One short email asking for what you need is worth more than a perfect one you never send. Cut anything that does not fit you.

Email 2  ·  Accommodation for an appointment

Use this when you have an appointment or intake coming up and you need something in order to attend. Shorter than the full notice, and quicker to send.

I have an appointment booked for [DATE] at [TIME] with [NAME OF ORGANISATION]. I am writing to ask for accommodation of my disability, for that appointment and for anything that follows from it. What I need in order to take part: [Delete anything that does not apply to you. Add anything that is missing.] - Enough notice of any appointment to arrange transport and any care I am responsible for - Help with transport, or an appointment at a location I am able to reach - To bring a support person with me - Information given to me in writing rather than only spoken aloud - Appointments at [TIME OF DAY], because of my condition - Breaks during longer sessions - [ANYTHING ELSE YOU NEED] Please confirm the following in writing: 1. Asking is not refusing. Requesting accommodation is not the same as declining to take part, and it should never be recorded as though it were. If anyone suggests otherwise, say in writing: this is a request for accommodation, not a refusal to participate.

Use this on its own if phone calls are a barrier for you. It is short on purpose.

Email 4  ·  No reply by your date

Use this if the date you set has passed and nothing has come back. Send it as a reply to your own original email so the whole thread stays together.

Reply to your own sent email. Open your Sent folder, find the original, and hit reply. That keeps the original message, its date, and your follow up in one thread. If it ever matters later, the whole story is in one place.

Where to send it

Always use the return address printed on any letter you have received. If you do not have one, there are no office-specific inboxes — every AISH and ADAP office routes to one of four zone inboxes, confirmed from the government's own contact page in August 2026.

Ask for the email address for written correspondence about your file, and ask them to email you while you are still on the line so you have it in writing.

After you send anything. Screenshot the sent email showing the date, the time, the subject line and the address. Save any automatic reply. Keep it all in one folder. That folder is the thing that settles arguments later.
